The ARLIS/SE Regional Conference will be held from November 8-11, 2001 in Fort Lauderdale and Boca Raton, Florida. Veterans' Day is on Monday, November 12, 2001, so some of you may have off and wish to extend your stay an extra day.

This registration form is provided for you to fill out. You cannot register online for the conference with this form. Use the form below to fill out all of the information requested. Then, print out the form and mail with your check or money order payable to ARLIS/SE. You will be given a receipt with your conference packet when you register onsite. Incomplete registration forms may be returned. Mail this form and your check or money order by Monday, October 1, 2001 to:

NOTE
Conference attendees do not have to be registered members of ARLIS/SE, although members of ARLIS/SE will pay a slightly reduced conference rate. If you wish to join ARLIS/SE or renew your membership in ARLIS/SE, you may do so by submitting your annual dues with your conference registration fees.
